Terms of Service This Terms of Service is made between the above mentioned customer and Keith's Pest Solutions. The terms of service is for 90 days, and will automatically renew itself every 90 days thereafter. Both parties agree to each other that Keith's Pest Solutions will be the pest control for the aforementioned customer. If for some reason the customer is no longer wishing to continue service with Keith's Pest Solutions, then the customers agrees to notify Keith’s Pest Solutions 30 days before the next visit. The customer agrees to make the property available to Keith's Pest Solutions for the regularly scheduled treatments. If the customer is not at home on the preset appointment, then Keith's Pest will treat the outside. This agreement does not imply that all insects will be eliminated. Various forms of an Integrated Pest Management system will be used, which means that that fewer chemicals may be used, and that other forms such as non-chemical Pest control may be used. Customer must understand that some insect activity may still be present and that Keith's Pest is not or will not be held liable for any damages caused by the chemicals used to control the Pest, or the damage done by the Pest. Customer acknowledges that when working with Organic / Botanical Pesticides that the time of results may be slow, and that if quick results is wanted then synthetic Pesticides must be used, and that regular treatment of the property is essential. Keith's Pest will use only properly labeled chemicals with an EPA Registration number, unless the Pesticide is exempt from E P A registration, then the directions provided by the manufacture will be applied as directed by the label. Keith's Pest will also from time to time make recommendations about conducive conditions for insect infestations. The customer also agrees to correct any conducive conditions to help prevent future infestations. Keith’s Pest will offer the option of pre-scheduling your quarterly treatment. The cost of the Future treatments will be determined by the pricing listed on "My Services Page". Customer also agrees that this agreement covers only the following Pest{s}, (listed below) while other Pests may be treated for, they will not be warranted. If additional treatments are needed, and any conditions that support insect activity has been resolved then the additional treatment will be free of charge. Keith’s Pest will also include Termite Protection after one year of continued service. The treatment on this agreement will be an inspection based spot treatment of any active termite areas. Keith’s Pest is not responsible for any damage done by insects or the chemicals used to treat them. If the customer misses any treatments then the First time service charge may be charge to restart service. And Termite coverage may then be null and void. And the one year will be reinstated. While Keith’s Pest will attempt to keep your Pest Control cost a low as possible, from time to time economic factors may result in an increase of the treatment price, without prior written notice. Treatment coverage on a General or Quarterly treatment includes the Structure and Three feet out. Service will be performed on the following months. Fire ants and any other exotic Ants in the yard will not be covered. There are options available to help curtail some ant species, such as Fire Ants.
Service will be done on a pre-scheduled basis and will fall on a day and week of the month that service is due, about every 90 days

********************************************************************************************************************************************************************* CONSUMER INFORMATION SHEET The structural pest control industry is regulated by the Texas Department of Agriculture (TDA), Structural Pest Control Service (SPCS), PO Box 12847, Austin, TX 78711-2847. TDA licenses the businesses, certified applicators and technicians who perform structural pest control work. Certified applicators and technicians must pass a written examination in order to receive their licenses. Pesticides must be registered with the United States Environmental Protection Agency (EPA) and TDA before they may be used in Texas. EPA registration is not a finding of product safety. Pesticides are designed to kill or control pests. Your risk of harm depends upon the degree of your exposure and your individual susceptibility.
Specific health and safety information varies between pesticides and types of exposures and is available on the label information or MSDS sheet, which can be supplied to you upon request from the licensed applicator. Take precautions when a treatment has been performed to avoid exposure to vulnerable individuals. Pesticides may be harmful if swallowed, inhaled, or absorbed through the skin. Avoid breathing dust or spray mist and any unnecessary contact with treated surfaces. If you desire specific information on precautions, refer to the pesticide label. The law requires that the application procedures specified on the label be followed. If you have questions about the application, contact the business or person making the application. If you suspect a violation of the law regarding structural pest control, contact the SPCS. In case of a health emergency, seek immediate medical attention. Pest Control signs must be posted prior to treatment in many instances. The signs should be posted in an area of common access at least 48 hours prior to treatment. The information sign will allow you to contact someone who can tell you what pesticide is being used.
If you are contracting for pest control services due to a home solicitation, you have the right to cancel the contract within 72 hours. You may exercise this right by notifying the pest control company that you do not wish to receive their service. In order to minimize the reliance on pesticides and reduce pest populations, a Reduced Impact Pest Control operator may recommend that you consider the sanitation or physical alteration of your work place or residence. It is your responsibility to follow those recommendations. Your pest control operator may or may not offer these services upon request. A proper inspection will provide the information necessary for you to choose the method of pest control which best suits your situation. Many pest problems can be solved without using pesticides.
This Reduced Impact Service will include an inspection report and treatment recommendations. You should review these and keep a copy for your records. Your cooperation in following the recommendations made by your service provider is essential to a reduced impact service program.
Pesticides may be used in a responsible and professional manner in a reduced impact pest control service. If you do not want a specific pesticide used or any pesticides used at all, you must note this in writing on the contract prior to the initiation of the service. If any specific pesticide or class of pesticides are not excluded, it may be used by the provider.
